Bronze Package
Free
Based on your outlined needs we will provide you with 2 local care options that suit your budget, are reputable, and have capacity for you or your loved one. We will discuss each service and help you choose which one you feel is best.
We will then assist you with setting up the service with your chosen provider.
Silver Package
£99
As above plus; Full care plan and identify further needs where neccessary, eg OT referal, medication independence support
We will arrange the set up of care with your chosen provider and ensure a smooth transition.
Gold Package
£149
As above plus; arrange hospital discharge , liaise with Occupational Therapist, Pharmacist, care provider, GP, Local Authority, Equipment hire/purchase where applicable. Inclusive of a follow-up check in at 2 weeks and 1 month to ensure the person using the service is satisfied with their situation.

Book a call, we can help you think about what steps you need to take and answer any questions you have to ensure you have peace of mind for later life.

We will take the £99 fee off any of our packages should you go ahead with a package above.
Additional services
£30 per hour
Sourcing an Occupational Therapist, Mental capacity assessments, new GP, Dentist or Physio.
Pharmacy Set-up e.g blister pack, Meal Delivery services, mobility aids and equipment.
Medication Independence Review.
Organising social clubs or activities.
